You can opt out of Single Check at any time as long as none of the following conditions are true:

1. You have an unpaid Capital Loan.
2. You have an unpaid third-party commission advance.
3. You have redeemed Walled Rewards on any unclosed transactions.

If any of the above conditions apply to you, please wait on submitting your form until the condition has cleared.

Opt Out of Single Check Request Form

• Input your First Name (before change)
• Input your Last Name (before change)
• Birthday Date (this is required to help us validate your profile)
• reZEN Business Phone # (this is required - it's a secondary method to validate your profile)
• State or Province of your primary license
• Description - type in Opt Out of Single Check
• When you submit the form, you will see a message saying the form was not submitted and that you will need to verify your email address. You will need to go to your email and make that verification for the form to be submitted for completion.
