To opt in to the Stock Purchase Program, please fill out this form to include the following:
- Your First & Last Name as it is in reZEN (middle name is omitted, even if you have it in reZEN)
- Birthday Date (this must match what you have in reZEN. This is how we validate your profile. If you're missing your Birthday date in your profile, please add it; it is required to complete this request).
- reZEN Business Phone # (this is a secondary method of validating your profile).
- The State or Province of your Primary License.
- Pick Opt In or Opt Out from the dropdown menu.
- In the Description input, either SPP Opt In Request or SPP Opt Out Request.

When you submit the form, you will see a message saying the form was not submitted and that you will need to verify your email address. You will need to go to your email and make that verification for the form to be submitted for completion.
- Shortly after you submit your form, you will receive an email letting you know that the Opt In or Opt Out addendum has been sent for you to sign.
- Please find the addendum in your email and sign it.
- Once the signed addendum is received, the change will be made in your reZEN profile.

Please be sure that you recalculate any transactions that have not yet been closed out in reZEN once you have been opted in or out.

Here's a brief video and step-by-step instructions:
- Log in to reZEN and open the transaction you need to edit.
- Click Edit Transaction. This will open up a module.
- Click Recalculate Commission. A message will pop up asking you to confirm. Please click Confirm.
- If your commissions no longer add up to 100%, please scroll to the Payment Participants Module and Adjust % / splits to fix this.
- Once you make the change, the transaction will need to be approved again. Please review everything and click Approve Commission Calculation to move the transaction forward.
