If you have any issues with your T4A, please report it immediately so we can review it and, if applicable, get it corrected for you ASAP.
Step 1: Login to reZEN, click My Dashboard on the left side, then click the Overview tab.
Step 2: Scroll down to the Tax Forms section and click Report a T4A issue. If you have multiple T4As and the issue only pertains to one, click the correct one.
Step 3: Tell us what's incorrect. Please be as specific as you possibly can:
- Which amount is incorrect (e.g. Net Commission, Revenue Share Paid)
- How much it's off by (i.e. what your records state).
- If you have any thoughts on what's causing the discrepancy.
Once you have a clear message, please click Send Email.
Once you send your email, our Support Team will look into the issue. We might ask you for backup documentation and then work with our Accounting team to resolve the issue.
