This document applies to agents licensed in Alaska, Colorado, Idaho, Iowa, Kentucky, Louisiana, Mississippi, Nebraska, New Mexico, North Dakota, Rhode Island, South Dakota, Tennessee, or Wyoming. Please see Additional Required E&O Insurance for more informaiton.
To submit your E&O Certificate of Insurance (COI), please use this form and see the steps below.
Select Issue Type:
- Under Issue Type: "Please choose your issue below", select: E&O Policy Required Documents
- In the Description field, enter: E&O: Certificate of Insurance
First Name, Last Name, and Birthdate
- Enter your First Name and Last Name exactly as it appears in reZEN.
- Enter your date of birth.
Note: Do not include your middle name or middle initial.
Email Address
- Enter the same email address associated with your reZEN account.
State of E&O Policy
- Enter the state for the E&O policy being submitted.
Note: Submit a separate form for each state if you have policies in multiple states.
E&O Policy Carrier/Provider Name
- Enter the name of the insurance carrier or provider that issued the E&O policy.
Note: You will see two options in this dropdown: State Group E&O Plan (RISC) and Other Equivalent Coverage. If your E&O policy shows CNA or RISC (Rice Insurance Service Center), select: State Group E&O Plan (RISC). If your policy does not show CNA or RISC, select: Other Equivalent Coverage
E&O Policy Number
- Enter the policy number exactly as shown on the Certificate of Insurance.
E&O Policy Start Date and End Date
- Enter the effective start date of the E&O policy.
- Enter the expiration date of the E&O policy.
Upload Certificate of Insurance (COI)
- Upload the Certificate of Insurance (COI) for the E&O policy.
- Verify that the document clearly displays:
- Policy Start Date
- Policy End Date
- Limits of Liability
- Deductible Information
- Requirements
- Upload only one attachment, preferably in PDF format.
- Ensure the document is clear and legible.
- Do not submit blurry, incomplete, or unclear documents.
Note: Failure to provide a COI attachment may result in the request being rejected and the policy not being updated in reZEN.
Upload a single attachment only. Multiple-page documents are acceptable, provided all pages are included in one file, and your Certificate of Insurance is included in the document package.
Submit Request
- Review all entered information for accuracy and submit the request.
- A member of the support team will review the submission and respond as soon as possible.
When you submit the form, you will see a message saying the form was not submitted and that you will need to verify your email address. You will need to go to your email and make that verification for the form to be submitted for completion.
You will get a notification via email shortly after submission and verification confirming that your Certificate of Insurance was received and added to your profile. Please keep an eye out for this message; should any problems arise, our team will email you with further instructions.
