I have unauthorized charge(s) on my card.

If you notice a charge(s) on your card that were not made by you or authorized by you please call the number on the back of your card. 

If you would like to file a claim due to a refund not received, item never arrived, duplicate charge, etc. please also contact VISA.

Visa will open an investigation on the unauthorized charge(s). You may be asked to provide the following:

• Name of Merchant

• Date of Disputed Transaction

• Amount to be Disputed

• Details regarding the disputed transaction (e.g., I have never bought anything from this merchant, wrong amount, etc.)

• Attach any documents that might be relevant to the case (receipts, communication, etc.)

• Location of the Card (In my possession, Lost, Stolen)
